Progress notes track how a client moves toward the goals set in the treatment plan across sessions. They’re updated after each contact to document what interventions were used, how the client responded, any barriers encountered, and any adjustments to the plan. This ongoing recording creates a clear, running record of progress toward specific goals, making progress notes the most appropriate choice for monitoring day-to-day advancement in treatment.

Discharge summaries are written at the end of treatment to summarize overall outcomes and future recommendations, not the ongoing work. Assessment notes focus on the initial evaluation and current status, rather than continuous progress toward goals. Clinical notes can describe client contact in general, but they do not inherently emphasize monitoring progress toward treatment goals the way progress notes do.
